Financial Responsibility Policy

You understand that you are obligated to ensure that our fees are paid in full.

  • If not utilizing insurance, payment is due at the time of booking.
  • If using insurance, you may be required to pay a co-pay either before or after the appointment.

We accept FSA/HSA (flexible spending/health savings account), all major credit cards, and debit card. A credit/debit card is required to hold your appointment. If you haven’t already, please provide method of payment in the client portal under “Invoices and Billing” and the tab “Billing Information.” Your sessions are NOT guaranteed until a method of payment is secured. Your card information will be encrypted and we will not be able to see the full payment information. You can change the card on file at any time through your client portal access.

In the case that you are sent an invoice for services, such as an insurance co-pay, your invoice is due within 7 days of receipt. If the invoice has not been paid within 14 days, you acknowledge that we will attempt to charge the credit card on file. A late fee of 25% of the service cost will be applied to overdue invoice payments that have not been paid or we have been unable to process after 21 days.

Balances unpaid after 90 days will be sent to a debt collection agency.

Due to the nature of our services, refunds will not be given for services rendered.

Cancellation Policy

Appointment reminders are sent via text and email 3 days prior and 1 day prior to the scheduled appointment time.

Please be advised that Your Kidney Dietitian PLLC has established a cancellation policy such that the Patient must give notice of cancellation or intent to reschedule at least 48 hours in advance of their appointment.

If canceling within 48 hours of the appointment time, the Patient will be charged the full out-of-pocket fee for their appointment. This fee will be charged to the card on file on the scheduled date of service. Please note that insurance providers do not cover or reimburse for any late cancellation fees. For pre-paid sessions cancelled within 48 hours, the session will be forfeited.

A no-show is defined as the Patient not signing in within the first 10 minutes of the scheduled session. No-shows will be charged the full out-of-pocket fee for their appointment. This fee will be charged to the card on file on the scheduled date of service. If the session was pre-paid or part of a package, the session will be forfeited. Insurance does not cover or reimburse for cancellation or no-show fees.

Insurance Plan Coverage Policy

We are currently in-network with the following commercial insurance providers:

  • UHC (including UMR)
  • BCBS (including Anthem)
  • Cigna,
  • Aetna

We also accept the following Medicare plans:

  • UHC (including AARP plans)
  • BCBS
  • Aetna
  • Original Medicare
  • Railroad Medicare

If you provide your insurance information, we will bill your insurance company for your session(s). It is your responsibility to verify coverage with your insurance provider.

For non-covered services, you are eligible to use FSA/HSA to pay for services. A Superbill for possible reimbursement by your insurance plan can also be provided upon request.

Discontinuation of Services

You agree that the practitioner, Kate Zalewski RDN at Your Kidney Dietitian PLLC, may discontinue services to you upon notification to you in writing for any reason, including the following: your failure to cooperate to the best of your ability in the activities and schedules planned; non-payment of fees within 21 days of the due date. After 3 cancellations/no-shows, the practitioner reserves the right to drop the Client.

If the Patient wishes to discontinue services with the practitioner, notice should be submitted in writing via email at kate@yourkidneydietitian.com.
